Please use the following checklist to gather the documents and information needed for the preparation of your individual income tax return. Not all items will apply to every taxpayer.
Personal Information
- Social Security numbers for yourself, spouse, and dependents
- Date of birth for yourself, spouse, and dependents
- Prior year tax return
- Bank account information for direct deposit of refund
Income
- W-2 forms from all employers
- 1099-INT (interest income)
- 1099-DIV (dividend income)
- 1099-B (proceeds from broker transactions)
- 1099-R (retirement distributions)
- 1099-SSA (Social Security benefits)
- 1099-MISC or 1099-NEC (self-employment or other income)
- K-1 forms from partnerships, S-corporations, trusts, or estates
- Rental income records
- Alimony received (for agreements prior to 2019)
- Gambling winnings (W-2G)
- Other income records
Deductions
- Mortgage interest statements (Form 1098)
- Real estate tax records
- State and local income tax payments
- Charitable contribution receipts
- Medical and dental expense records
- Unreimbursed employee business expenses
- Home office expenses (if applicable)
- Student loan interest (Form 1098-E)
- Tuition and fees (Form 1098-T)
- Casualty and theft loss records
- Investment interest expense
- Alimony paid (for agreements prior to 2019)

- Child care expenses (provider name, address, and EIN)
- Education credits (Form 1098-T)
- Retirement savings contributions
- Energy-efficient home improvement records
- Electric vehicle purchase information

- Estimated tax payments made during the year
- Any IRS or state correspondence received during the year
